Eligibility

 

Eligibility guidelines for Macon Volunteer Clinic include:

  • Patients must live in Bibb County
  • Patients must be between the ages of 18-64
  • Patients must be uninsured (no Medicaid, no Medicare, no ‘clinic card’ and no private or employer provided health insurance)
  • 2007 income limits are: 1 person household – $24,424; 2 person household – $27,384;  3 person household – $34,344; 4 person household – $41,304. The income limit increases as household size increases.
  • Patients must provide a valid social security number

 

Services

 

Services include physician visits, non-narcotic prescription medication, laboratory analysis, x-rays, eye exams, limited dental care and cancer screenings such as mammograms, Pap smears and colonoscopies. All services are free of charge. We do not provide birth control.

 

Screening Hours

 

Patients must be screened before seeing a physician.  No appointment is needed for screening on Wednesday and Thursday afternoons from 2:00 – 4:00 p.m.  Please arrive before 3:30 pm and bring the following items to the screening:

·         Picture ID

·         Social Security card

·         Letter from your employer stating that you do work and you do not have health insurance

·        If your spouse is working, a letter from your spouse’s employer stating that there is no insurance that covers you

·        If you are self employed – most recent 1099 or profit and loss statement for the current period

·         Most recent payroll slip

·         Two utility bills with YOUR name and address (proving that you live in Bibb County)

·         Current year’s 1040 tax return

 

Appointment Hours

Appointments with a doctor are ONLY possible at these times:

·         Monday         5 – 7 pm

·         Tuesday        5 – 8 pm

·         Wednesday    9 – 12 noon

·         Thursday       9 – 12 noon 
Some Saturday apppointments are available, every sixth Saturday.

 

Appointments with a dentist or hygeniest are ONLY possible at these times:

·         Monday         1 – 4 pm
Only established patients who have seen a physician may schedule a dental appointment.



Office Hours

Office hours are:

·         Monday         12 – 7 pm

·         Tuesday        1 – 8 pm

·         Wednesday    9 – 4 pm*

·         Thursday       9 – 4 pm*

·         Friday           9 – 12 noon 
We are closed for lunch Wed. & Thurs.from 12:30—2:00
